Here is a list of documentation that may be requested
The following identification is required for each family member;
- Birth Certificate
- Driver's License
- Certificate of Native Status
- Passport
- Immigration Papers
- Social Insurace Number (SIN)
- Ontario
- Health Card (OHIP)
- Proof of rent or home ownership: lease, rent receipts, mortgage papers, property tax papers, condominium fee receipts, utility bills, land title and deed
Eligibility Verification Documents
- Pay stubs or written proof of income may be required, bank statements or passbook (must be up-to-date)
- Divorce/seperation papers, support agreements, proof or documentation of child care costs
- Employment Insurance (EI) stubs
- Proof of investments (RRSP, Bonds, Term Deposits, Shares, etc)
- Proof of self-employment and all financial information for your business: registration, business bank account statements,
- Proof of company assets, profit and loss statements, income tax returns, and doctors note if your physically or mentally ill and cannot work
- School records
The processing of your application may be delayed if you do not bring all of the necessary papers to your interview. A financial needs test is used to determine your eligibility for employment and Financial Support.
Eligibility is decided using the same Provincial and Regional standards and guidelines for all applicants. All information is kept strictly confidential and is used only to determine eligibility for Employment and Financial Support. Applicants will also be asked about their work history, work-related skills, and education. Applicants should also bring a current resume.
